Do you get rid of foam inside battery holes?

Yes. Remove the foam In the battery holes Before placing battery in the Hole. The battery replaces the foam.

How do you test button batteries with tester? Where do you place them?

These battery testers can be bought separately, I bought one a few years ago. The red thing on the side slides to any battery length, so you can put a small button sized battery up to a large D battery. Once side is for the positive and the other the negative. You can test the 9-volt battery (rectangle) as well, but its done a little differently.

Is the tester accurate?

the tester worked for a period of time then just quit working about a month ago, I threw it away and use a regular meter to check batteries but do like the organizer.
