Im 5'4, 36c cup and weigh 181 ibs. my pants run around xxl or 16, what size should i get?

Great question. I am pretty close to you size wise. I'm 5'4 179 38D. I purchased a lg. It fit well. Though I feel as if I could have gone to a medium and fit even better. So ultimately I would recommend for you a medium rather than large. The skirt was slightly loose but I improvised to make it work. All In all it is such a beautiful outfit.

What’s the difference between one size and medium?

Not sure I went with a size large .

What color is the underskirt for the army green one?

For the army green one the underskirt is dark brown.

can you just purchase the skirt or not?

You can look up medieval skirt and see, on this one they come as a pair.
