Too many different slim extreme 4d options. which one is the best?

They are all basically the same. This one heats up intensely if used as directed. Feels like your skin may be on fire.

It doesnt specify if you must only apply like a lotion on the troubled areas or if you must use a wrap or a sweat vest over it also. Help!?

I wouldn't use a wrap or vest with it because it does get pretty hot. I wear tight high waisted cotton leggings with mine. I used it once with a waist trainer and regretted it almost immediately. It got so hot that my skin was pink and had raised red welt like burns for a few hours after use.
