Does this come with the handle already attached?

Yes, the handle is attached. It was much smaller than it was described and I returned it.

Can this be heated on a stovetop or does it hold hot water?

My suggestion is to not heat it on a stove top and use it to hold hot water and pour over the infuser that holds your tea. That's what it is there for. I have one like this from meijer and it clearly says to not heat this up on a stove. It can make you sick with repeated use.
