The fabric is fused to the foam cups. And: all foam (whether molded or cut and sew) has a thin layer of fabric over it.

Q: Can anyone tell me what the D/DD size measures side to side and top to bottom?

I don't know the answer because I have already used them in a customers dress. I can say they worked well for her size D cups.

Q: Would these work in a halter top bathingsuit?

I used this item in a sundress between the dress and the lining. I didn’t get the dress wet while wearing it. The item description says for swimwear. The trick would be how to attach it to the swim suit. Some suits have the top lined and a slit to insert an item like this. It would work well for that type of suit. If the suit is unlined, it would be hard to attach it.

Q: how are these positioned? Curve UP or Down?

Generally I stitch them in curve up. But you can choose to do either.
