No, it comes with one stick of lead in the pencil, but no additional refills.I personally like to get softer (darker) leads, e.g. http://www.amazon.com/Staedtler-Mars-Carbon-Lead-200-2B/dp/B003BIEZRW/

Q: Is there a way to get the lead out of the cap from the 2mm lead holder?

I'm not sure I understand the question. Did the lead break off in the "cap" lead sharpener?

Q: Should you twist the sharpener clockwise or counter-clockwise? somehow it seems twisting counter-clockwise (screwing the lead out) works better.

Thank you for your interest in our Rotring brand. By default the sharpener should work by twisting the lead clockwise.
