Does this item wear out with usage? or can it be used repeatedly for years?

I bought this 2 years ago and it seems to still be working, although I only use it as a water meter. I don't worry about the Ph of my soil.

I do not know what the manufacturer reccomends, but as the unit is powered by the electrolytic action of dissimilar metals then no. To monitor for moisture with minimal effect on the reading or equipment, you need something using ac to drive the electrodes.

Can this meter measure ph level in water alone?

No, because it is calibrated to a particular soil, most likely not yours and obviously not water. The readings are a function of both acidity and concentration, while concentration of, for example, humic acid changes in the range of 0.01-0.2%, which is 200x. Don't buy this class of testers for ph measurement! Litmus paper will do the job.
