The size is measured by height. the extra small has a handlebar height of 15.5"-19.5" while the small has a height of 19"-25". I would probably recommend the small over the extra small.

Q: Can it be used as wheelchair when seat is down?

Q: If using as stability confidence builder only, can you walk with a normal rhythmic gait?

My daughter has used this type of walker for years she has always been able to walk with a normal gait. Make sure you pay attention to the width so you have enough room to achieve the normal gait.
