Q: what is the diameter of these pizza crusts?

They may have been around 12 inches in the past, but it looks like they've decided to take the route of many food producers and shrunk the size of the crusts. I've bought the sprouted grains crusts many times and this last batch measured around 10 1/2 inches which is a considerable reduction in size.

Q: Can these be baked on a pan instead of directly on the oven rack?

We would not advise baking on a pan, it loses it's crispness.

Q: Why is this so expensive when it’s 4.99 in the store?

Probably Amazon markup due to shipping costs and a small charge going to Jeff Bezos - he did suffer a bit of a hit with his recent divorce.

Q: Are they sold individually or in a case? How many packs in a case?

There are 3 crusts to one pack. That is how I bought them a couple of months ago.
