The longer side goes to the back for female. For male, the longer side goes to the front. Hope this help.

Can this be used with pants on?

you will have to pull down the front of the pants to be able to insert the device...

Would it be possible to rinse this and spray it with alcohol for extra disinfection while camping? (No soap option available?)

According to the manufacturer, it can be "rinsed and cleaned with normal hand sanitizer" which is mostly alcohol, so that shouldn't be a problem.
