Q: Would the pockets be large enough for camera flashes, and would the d rings support the weight of dslr camera's?

Yes I believe flashes would fit it 4 of the front pockets and 2 inside pockets. It's very well constructed. Supporting DSLR cameras woud probably be uncomfortable on the neck for long periods of times but again I think the rings would hold the camera. There are two front breast level hook and loop fasteners that are also very strong. I've been using my vest for 2 years in all sorts of strenuous environments, stretching and pulling on all aspects of the vest and nothing has failed yet.

Q: How well does this item breathe? I'm a professional photographer who may need to wear this for many hours while shooting.

It's not 100% cotton so it doesn't breathe as well as I would like.I wear mine every day, year round, but it does get uncomfortable if it gets in the 80s. The sleeveless design really helps but it's not air-conditioned.It's just so hard to find 100% cotton, so this might be the best choice.I'm a big guy. I needed a 3X so I felt this was my best bet. I think that if I could fit into a 1X I'd have more options with a lighter weight 100% cotton.!!!.

I can't speak to the Small and your size directly, but just reiterate what I experienced. I am male and usually wear an L in shirts and jackets. The M fits loosely on me, although that's a good thing because I can wear it easily over a shirt and heavy sweatshirt on colder days.

Q: Is this vest waterproof

No it is not. I don't know of any vests that are completely waterproof. There are products on the market you can use to spray-on your vests or other clothes or shoes that make them water-resistant; that is repel rainwater or snow.
