Why does it state that a lithium battery is required if it’s 220v

It does not make sense that a battery is required, since, as you stated, this product plugs directly into the mains. I assume it is a mistake on the listing.

Does this cut through concrete slabs and tiles?

Yes, depends how thick they area of course, but I used it with a diamond disc to cut through slate cobbles about 2 inches thick (need to cut on both sides) and it was absolutely fine. Just need to make sure there is plenty of water coming through the machine and on to the stone - but not too much - just a good trickle.

Would this cut through marble for back of a fire

This item is listed as being able to cut Granite which is a harder stone than Marble so it will be suitable for your intended work.

Where is the water outlet? I think is defective and does't have one as I've pumped up the water and no water comes out of the unit.

It's at the back of the blade. There is a small valve that controls the water flow. I cut at least 10m of 20mm quartz worktop with this machine and it cuts very well if you spend the money on decent blades.
