I appreciate this is an old question but I wanted to provide an accurate answer. The order of filtration depends on the type/model of filter you have. Some flow water from the top down, others from the bottom up. You should use coarse sponge first to mechanically filter large bits of diet e.g. plant leaves. Then your fine sponge/floss as this is also mechanical filtration. Next is your chemical filtration like carbon (but you should only use this for removing medication; it's not necessary if you set your tank up correctly). Finally your biological filtration I.e. ceramic rings.

Q: Do I actually need these for a cold/fresh water tank?

They are a good addition to any filter cold or fresh water, as they provide a large surface area that nitrifying (good bacteria) can attach to, to reduce dangerous ammonia (urine) excreted by fish. However frequent water changes will also help whether or not this media is used in your tank. Google the nitrogen cycle in aquarium for more details. Hope this helps.
