Yes. Shouldn't be a problem. It has two different orientations for enhanced compatibility

Q: Will this work with i7-8700k oc to 5ghz on a small itx motherboard in an htpc case?

Noctua make some of the best CPU coolers on the market. This low profile unit is a superb fit for htpc's. I have built one myself using it. Buy it.

Q: Does this use the stock backplate that comes with the motherboard? I really dont want to have to disassemble it all.

If your stock backplate is made of metal and has threads in it where the cooler can either be bolted on or the saddle round the Cpu can be unbolted then you will be able to bolt this down onto the existing backplate. If your motherboard has a plastic backplate or the saddle around the Cpu that your stock fan clips to is plastic and has those plastic clips that go through the motherboard to hold it together then the only was is to dismantle board from case, remove plastic stock plate and fan retainer cradle and fit the metal components that come in this kit
