Q: What size pipe fitting is needed to connect conduit to this cap?

Mine fit on an existing 1" conduit.

Q: does it come with a rubber gasket?

yes it does come with a rubber "crush type" gasket

Q: My well pipe has a 5 5/8 OD, will this fit?

I am quite certain that it will not.

Q: Is this a vermin proof cap?

Yes. It has a rubber gasket seal that compresses around the well as you tighten the bolts, and is vented by a screen. The screen has smaller holes in it than a window screen.
