Regulate your input voltage.
Set this up on the bench and the signal looks good. No clipping induced until the supplied voltage dropped below 10v. Id recommend keeping it at 12+ though. During the testing I noticed that a 1v increase in the supply voltage would add a DC offset voltage to the output signal temporarily. I installed this in my car without regulated voltage and i'm noticing a moving DC offset on the output signal because of the fluctuating input voltage. Ill be using a step up converter to feed this preamp a constant 15v.

Very Lo Noise
Board is compact and has a full ground plane. It is very low noise and works fine. I used it to boost the signal to my subwoofer which it does perfectly. Note that it would be difficult to mount with screws or metal standoffs due to space and the possibility of shorting something out. I used a piece of double-backed tape, but plastic snap standoffs work likely work. In the pic you see that I added a board in my metal box for a 1000uF capacitor, LED, and power jack---would probably be just fine if you hardwired directly to a decent 12vdc wall wart. Very nice board.

Good little preamplifier
I built an aux input adapter for my car's factory radio but it just wasn't loud enough. Added this to the system between the jack and the radio and now the levels are just about perfect.Runs great off unfiltered automobile 12v with no alternator whine or ground loop issues. The audio quality is good.

A basic audio building block.
These are tiny and work on a single supply. You get a dual op amp wired as 2 inverting amplifiers. As others have pointed out, you can multiply the gain by chaining both stages together, but beware the low pass filter effect will also be multiplied into a 2 pole filter. Be prepared to remove or replace the capacitors in the inverting feedback loop. You can build this yourself easily but sometimes a compact ready-made preamp is just what you need. This is not configured for microphone amplification, nor is it able to drive a loudspeaker.
