Q: How easy is it to line up and screw on the cap in the dark?

Very easy. The screwtop has thick threads that line up easily. The cap also glows in the dark. As to line yourself up, well I've never had any problems.

Q: Is the screw top tethered to the bottle???

Yes it is attached to the bottle so you can't loose it.
